Telehealth Informed Consent

This document explains what telehealth is, its benefits and limitations, and your rights as a patient receiving care through telehealth from Attending Medical Group, PLLC (the "Practice"). You will be asked to acknowledge this consent inside the secure patient platform before your first visit. A copy is published here so you can read it before you enroll.
What telehealth is
Telehealth means receiving medical care remotely, using secure video, audio, and messaging technology, rather than in an in-person office visit. Through telehealth, your clinician can take your history, evaluate your condition, order laboratory tests, prescribe medication when clinically appropriate, and provide ongoing care and monitoring.
What to expect at this Practice
- Your care is provided by a licensed clinician — a physician or a nurse practitioner — who is licensed in the state where you are located at the time of your visit.
- Your initial evaluation is a live video consultation. This Practice does not prescribe based on a questionnaire alone.
- You will have a dedicated clinician who manages your ongoing care. Between visits, you can message your clinician's team through the patient platform.
- Where state law requires physician supervision or collaboration for nurse practitioner care, the Practice maintains those arrangements, and your care follows clinical protocols developed and supervised by our medical director.
Benefits and limitations
Telehealth improves access and convenience and, for the conditions we treat, supports care that is comparable to in-person management. It also has limitations you should understand: your clinician cannot perform a hands-on physical examination; care depends on the accuracy and completeness of the information you provide; technology can fail or lose connection; and some conditions cannot be safely evaluated or treated remotely. If your clinician determines that telehealth is not appropriate for your situation — at enrollment or at any point in your care — they will tell you, explain why, and help direct you to an appropriate setting for in-person care. You will be told "no" if medication or remote care is not right for you. That is part of the service, not a failure of it.
Emergencies
Telehealth is not for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use the patient platform or the adverse-event reporting channel to report urgent symptoms — those channels are not monitored continuously.
Your responsibilities
You agree to: provide accurate, complete, and current information about your health, medications, and allergies; be physically located in a state you have identified to us at the time of each visit (your clinician must be licensed where you are); participate in follow-up visits and monitoring your clinician recommends; and use your own identity — care is personal to you and prescriptions may not be shared.
Prescriptions
Any prescription is issued only after clinical evaluation and only where your clinician determines it is appropriate. Prescriptions are sent electronically to the pharmacy of your choice. Medication has its own risks and benefits, which your clinician will review with you, and no outcome is guaranteed. You are free to fill or not fill any prescription and to seek care elsewhere at any time.
Privacy and technology
Your visits and records live in a secure, HIPAA-compliant patient platform. Your health information is protected as described in our Notice of Privacy Practices. Video visits are not recorded by the Practice, and you agree not to record visits without your clinician's consent. You are responsible for using a private location and a secure device and connection for your visits. Technical requirements: a device with a camera and microphone and a broadband or cellular data connection.
State-specific provisions
Some states impose additional telehealth requirements — for example, specific consent language, identification of the clinician's license, or follow-up care provisions. Where your state requires more than this document provides, those additional terms will be presented to you in the platform and form part of this consent. You may request the name, license number, and licensing board contact for any clinician who treats you.
Costs
Membership fees, what they include, and what is billed separately (such as medication and laboratory services) are described at /pricing and in the membership terms you accept at enrollment. Telehealth services at this Practice are provided on a direct-pay basis; we do not bill health insurance for visits.
Your rights
You may withdraw this consent and stop using telehealth services at any time, without penalty beyond the membership terms you agreed to. Withdrawal of consent will not affect care already provided. You have the right to ask your clinician questions about anything in this document, and to receive answers before care begins.
Acknowledgment
By acknowledging this consent in the patient platform, you confirm that you have read and understood it, that your questions have been answered, and that you voluntarily consent to receive care from the Practice through telehealth.
