Reporting an adverse event

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use this channel for urgent symptoms. It is monitored during business hours only.

If you experience a side effect or an adverse reaction to a medication prescribed through the practice, report it to care@myattending.com or (904) 906-3443 during business hours. We document every report and route it to your treating clinician.

You may also report an adverse event directly to the FDA through MedWatch, at 1-800-FDA-1088 or fda.gov/medwatch. Reporting to us does not prevent you from doing so.
